If you've been considering a lip treatment, you've probably encountered both options and found the choice confusing. They're often marketed as alternatives to each other, but they're actually very different treatments suited to different goals. Here's a clear breakdown.

The lip flip: what it is

A lip flip uses 4–6 units of Botox injected along the upper lip border. This relaxes the orbicularis oris muscle just enough to cause the upper lip to gently evert (roll outward and upward), revealing more of the pink upper lip surface. The result: the appearance of a fuller, more defined upper lip — with zero volume added.

Crucially, a lip flip doesn't change the actual size of your lips. It reveals more of what's already there. The change is subtle — typically 1–2mm of additional upper lip show — and it's most visible from the front and in three-quarter profile.

Lip filler: what it is

Lip filler uses hyaluronic acid injected directly into the lip tissue to physically add volume, shape and definition. Results are immediate, more dramatic (depending on the amount used), and last 6–9 months. Filler can be used to add volume to the body of the lip, define the vermillion border, enhance the cupid's bow, or correct asymmetry.

Volume change: The lip flip adds no volume. Filler physically adds volume.

Duration: The lip flip lasts 6–8 weeks. Filler lasts 6–9 months.

Cost: The lip flip is significantly less expensive, using only a few units of Botox.

Reversibility: Both are reversible — Botox wears off, and HA filler can be dissolved with hyaluronidase.

Recovery: The lip flip has minimal recovery. Filler typically causes swelling for 2–5 days.

Who should choose the lip flip?

The lip flip is ideal if you want a subtle enhancement and are nervous about committing to filler; if you have a tendency to show very little upper lip at rest; if you want to try something before committing to more; or if you're happy with the volume of your lips but want more definition and a slight pout. It's also a great standalone treatment for reducing a "gummy smile."

Who should choose filler?

Lip filler is the better choice if you genuinely want more volume; if you have significant asymmetry you want to correct; if you want results that last longer than a couple of months; or if you want a more comprehensive reshaping of your lip proportions.

Can you have both?

Yes — and they're often combined beautifully. A small amount of filler for shape and definition, combined with a lip flip for the upper lip reveal, gives a result that's more complex and natural-looking than either treatment alone.
